Pre-Surgical Assessment and Testing
Pre-surgical assessment is a very important part of your surgical experience.
An Ohio Valley Registered Nurse will call you to gather information about your medical and surgical history and medications. The nurse will confirm your procedure, instruct you about arrival time and remind you of any special instructions. The nurse will also help determine if you need additional tests before your procedure, which may include lab work, X-Ray and EKG.
To start your pre-surgical assessment, please submit your online medical history questionnaire. Please complete and submit your questionnaire as soon as possible. This will help your pre-surgical assessment nurse with organizing and documenting your medical history, and help us ensure that your procedure is as safe as possible.
Be sure to have the following information available before filling out your online medical history questionnaire:
- The names of your physicians.
- A list of all medications you are taking, their dosage and frequency, including non-prescription.
- A list of surgical procedures you have ever had, and the approximate dates.
- The name of the surgeon who will perform your upcoming procedure at Ohio Valley Surgical Hospital.
